Don Mills Family Health Team is an innovative provider of primary health care programs and services for the residents of Don Mills and surrounding neighborhoods in Toronto. Family health teams are an initiative funded by the Ontario Ministry of Health and play an important role in providing access to quality healthcare services. As a family health team, we are established to ensure that our patients receive a full range of healthcare services and health promotion programs in the local community.

We bring together different health care professionals to co-ordinate and deliver the highest possible quality of care to you – our patients – and to promote healthy living. At Don Mills Family Health Team, we are fully committed to supporting the health and well-being of our patients.

is to be a leading community-based primary health care organization— providing innovative, multi-disciplinary patient-centred care, emphasizing continuous quality improvement, health promotion and disease prevention, and focusing on social determinants of health.

Our Patient Policies

Privacy and Confidentiality

At Don Mills Family Health Team, it is our responsibility to keep your personal health information private and confidential. We ensure that our practices are in compliance with Ontario’s Personal Health Information Protection Act, 2004 (PHIPA) in order to protect our patients privacy while providing effective healthcare services.

Late Cancellations and Missed Appointments

If you need to reschedule cancel an appointment, we ask that you contact us to do so as soon as possible. We understand that there are times when you have to miss an appointment due to emergencies or obligations for work or family. However, when you are not able to call to cancel an appointment, this could prevent someone else from being seen by a provider of our health team. We sincerely ask for your understanding regarding the following late cancellation and missed appointment policy.

  • Any missed appointment without a 24 hour (1 business day) cancellation notice will be recorded in the patient chart.
  • First missed appointment: Verbal notice
  • Second missed appointment: Warning letter
  • Third missed appointment: $50 will be charged
