Position Overview

Welcare provides primary health care, including health promotion and disease prevention and chronic disease management to the population of mid-town Toronto. Our team is growing! We are looking for two proactive, clinical-minded RNs to join our team of 8 physicians and 4 administrative staff to work in a collaborative model of care. As our first nursing hires, you will play a foundational role in optimizing patient flow, enhancing chronic disease management, and ensuring our clinic operates at peak efficiency.
The Registered Nurse (RN) plays an integral role in the care of the patients. The role of the RN at Welcare requires a thorough knowledge of primary care nursing and the ability to function effectively in a team environment. The role also requires a high level of interpersonal skills to interact with patients and their families in a holistic, compassionate manner.
Clinical Responsibility
- Work collaboratively with physicians and interdisciplinary team members to manage the needs of patients in a fast-paced primary care environment.
- Conduct individual patient assessments, including patient history, recording of vitals, biometric measurements, and triage of patients based on acuity of concerns.
- Perform pre-screening activities including blood pressure, heart rate, weight, oxygen saturation, and other baseline assessments to prepare patients for provider visits.
- Administer routine childhood and adult immunizations, flu/COVID vaccines, and therapeutic injections (e.g., Vitamin B12, allergy injections, psychiatry medications).
- Provide wound care including dressing changes, suture/staple removal, and assist physicians with minor in-office procedures.
- Perform and document point-of-care testing, including urine dipstick testing and pregnancy testing.
- Review incoming laboratory and diagnostic imaging results, identifying and escalating urgent or critical findings to providers in a timely manner.
- Assist with prescription renewal processes by verifying medication dosages and pharmacy information prior to physician authorization.
- Share results of assessments and interventions with referring providers within established timelines.
- Ensure the maintenance of patient privacy, confidentiality, and accurate documentation in accordance with clinic policies and professional standards.
- Maintain meticulous, real-time records within the clinic’s Electronic Medical Record (EMR) system.
Chronic Disease Management & Preventative Care
- Participate in the development and delivery of targeted programs including chronic disease management, mental health and addictions, reproductive health, and child health initiatives.
- Support and coordinate chronic disease management (CDM) recall programs for patients with diabetes, hypertension, and other chronic conditions to promote adherence to recommended follow-up care, blood work, and preventative screening.
- Implement goal-oriented, time-limited intervention plans in collaboration with patients, families, and the interdisciplinary team.
- Provide patient education and coaching related to lifestyle modifications, diabetes management, home blood pressure monitoring, medication adherence, and health promotion.
- Identify and address preventative care gaps including cervical screening, mammography, and colorectal cancer screening during patient encounters.
- Evaluate and document outcomes of patient care interventions and participate in quality improvement activities.
Clinic Optimization & Coordination
- Oversee medical supply inventory, stocking of exam rooms, and maintenance of vaccine cold chain storage requirements.
- Collect data and support reporting requirements for statistical, quality improvement, and operational purposes.
- Assist in the development and implementation of clinical models, collaborative care guidelines, and workflow improvements as required.
- Work effectively both independently and as part of an interdisciplinary team while adapting to evolving clinic and patient care needs.
Qualifications and Requirements
- Bachelor of Science in Nursing (BScN) or equivalent Bachelor’s degree in Nursing.
- Current General Class registration and member in good standing with the College of Nurses of Ontario.
- Current Basic CPR certification.
- Previous experience in primary care, community health, chronic disease management, ER, or urgent care settings is considered an asset.
- Demonstrated clinical nursing skills including patient assessment, triage, care planning, implementation, and evaluation. Phlebotomy skills an asset.
- Knowledge of community resources related to health promotion, illness prevention, and chronic disease management.
- Strong communication, organizational, and multitasking skills with the ability to work effectively in a fast-paced environment.
- Demonstrated ability to work collaboratively within an interdisciplinary team while maintaining flexibility and adaptability.
- Strong computer and EMR documentation skills.
- Commitment to patient-centred care and a collaborative, team-oriented approach.
